How you can make a difference: As a Physical Therapist at Summit Health, you’ll deliver high-quality, evidence-based care in a setting that respects your clinical judgement and supports your growth. Key responsibilities include: Performing and documenting initial evaluations and treatment sessions in alignment with the state practice act and current best practices. Develop individualized care plans that include appropriate prognosis, functional short/long term goals and complete documentation in a timely manner. Regularly reassess patient progress and modify plan of care as necessary. Communicate effectively with clinic staff, physicians and other members of the health-care team.
