Trinity Health at Home is seeking a highly qualified registered
nurse to join our Delaware County team in a home health role. This
position works three 12 hour shifts/paid 40 hours. This position
will work weekends and either Friday or Monday and be responsible
for 4 start of care visits each day. Provide one-to-one,

RNs. Home Care RN position summary Provides primary nursing care to
home based care patients as ordered by the physician, and in
accordance to the organization’s policies and procedures. The RN is
responsible for the initial assessment and care plan development in
collaboration with the Population Health Case Manager. Provides
functional support/supervision to Home Healthcare Aides and LPNs as
needed. Provides therapeutic intervention, overall care management
and achievement of top decile client outcomes and patient

qualifications Graduate of an approved nursing education program
Licensure as a Registered Nurse in the state of PA One (1) year
experience as a professional care nurse in home health and
admissions Must have current Driver’s license and reliable
