Job Description:
We are seeking a dedicated Physical Therapist for home health
care in Rifle, CO. Provide individualized evaluations and
treatments based on physician orders, aiming to enhance patient
independence. Develop and regularly update care plans, reassessing
progress and modifying goals as needed. Collaborate with
interdisciplinary teams, communicate patient needs, and support
families. Supervise PT Assistants and Aides, ensure documentation
compliance, and participate in ongoing professional development.
Qualifications include a Bachelor's in Physical Therapy, an active
state license, CPR certification, with preferred experience in
